In general, we have as few rules (guidelines, policies, procedures, etc.) as possible. Subsequently, we develop these things IFF (If and only IF) they are needed And there is a sufficient probability that future repition is required, to justify the effort needed to write something down in an accessible manner.
One consequence is that the first instance of a problem generally requires a higher level of tolerance than a repeated occurance.
